Creating a Questionnaire

The Questionnaire Builder allows you to create additional questionnaires/assessments that you may use to manage your client’s application.

Accessing the Questionnaire Builder

To go to the Questionnaire Builder, click on "Settings" from the Main Menu and click on Questionnaires.

Click the "Create Questionnaire" button.


This is what the Questionnaire Builder looks like when you first access it. Let's go over these sections one by one.



Preparing the Questionnaire

You have the option to choose the Default Case Type that will be assigned to the case whenever it is completed through your embed link on your website, social media, etc. The Website Embed ID will be needed in another stage if you intend to embed on your website.

Tags are an easy way for you to categorize and keep track of your questionnaires. You will also be prompted to add Tags before you can add questions first, so be sure to add at least one.

Add a Title for your questionnaire, along with a description. You can also assign a Minimum Passing Score if you have a scored questionnaire.


Adding Sections

You may add Sections to separate the different areas of your Questionnaire. To do so, just click on the "Add Section" button on the right side of the screen.

You can add a title and description to each section of the questionnaire, along with points, if your questionnaire is scored. You can also Duplicate sections and enable "Allow multiple entries", which will allow the recipient to add multiple entries/answers to the questionnaire.


Adding Questions

Next, you may add your Questions by clicking on the "Add Question" button on the right side of the screen. These questions will be added to the "active" section, which is the section you may have currently selected. Just like sections, you have the option to add descriptions to your questions.


There are different types of questions that you can add to your questionnaire. Click on the "Select" button, and a drop-down menu will appear showing you a list of all the available question types.



Different question types will give you different options for responses as well. You can select whether you will allow only Alphanumeric / Numeric values, or just Phone numbers on the responses. You can also choose whether the question will be "Required" for the form completion or not.

Note: You can reposition both the Sections and the Questions after you have added them. Just click and drag throughout the questionnaire.

Don't forget to save your work by clicking on the "Save Changes" button on the upper right corner of the Questionnaire Builder page.


Additional Tools

1. Hiding Questions

You can Hide Questions while making your questionnaire. This will make navigating through the questionnaire easier for you. Just click on "Hide Questions" on the right side of the screen.


2. Add Rules

You have the option to apply custom rules to the questionnaire. To get started, select "Manage Rules" on the right side of the screen.

Here are the different rules/actions that you may apply to the questionnaire.


3. Choose Section

On the left side of the screen, you will see the "What's the grouping for the questions?" drop-down menu. It allows you to navigate through different sections in your questionnaire. Just click on it, and all the sections you've added to your questionnaire will appear.



Note: Whenever you add a new grouping to your questionnaire, be sure to click on "Reload sections" next to the "Choose Section" drop-down menu.
